By uploading one or more images (the "Images") to the Pain Research Foundation, Inc. ("PRF") website as part of the HOPE EXHIBIT project, I hereby:

• represent and warrant that I am the owner of all right, title and interest in and to such Image(s);
• represent and warrant that the Images depict only persons who are at least eighteen (18) years of age or, if the Images depict any person under age eighteen (18), I am such minor's parent or legal guardian and hereby agree to the terms of this Consent and Release on such minor's behalf;
• grant to PRF permission to use, adapt, modify, reproduce, distribute, publicly perform and display such Images, throughout the world, in any form now known or hereafter developed, by incorporating the Images into this website and/or publications, photo exhibits, audiovisual productions and/or other media or materials produced in association with the HOPE EXHIBIT project;
• understand that PRF reserves the right to determine whether and how to use such Images and that my Images may be used by PRF in its sole and absolute discretion;
• release and agree to indemnify, defend and hold harmless PRF, and its agents, employees and affiliates, from and against any and all claims that I or any third party may have now or in the future concerning rights of privacy and publicity, copyright infringement, defamation and any other cause of action arising out of or relating to PRF's use of such image(s);
• declare that I am at least eighteen (18) years of age and that I have reviewed this Consent and Release and am fully familiar with the contents thereof.
